(1)    APPLICATION FOR INDIVIDUAL MEMBERSHIP (WHITE FORM)

Complete the form and attach with the following: -

1. Copy : Trading License / PKK / PUKONSA / CIDB (if any);

2. Copy : IC;

3. Photo : 2 Copies;

4. Registration fee of RM10.00 and Annual Subscription fee of RM30.00.

(2)    APPLICATION FOR CORPORATE MEMBERSHIP - PTE LTD COMPANIES  (PINK FORM)

Complete the form and attach with the following: -

1. Copy : Memorandum And Article of Association;

2. Copy : Form 24 and 49;

3. Copy : Trading License / PKK / PUKONSA / CIDB (if any);

4. Copy : IC of Managing Director;

5. Certified copy : Certificate of Bumiputera status issued by Bumiputera Status Registration and Issuance Committee of Bumiputera Participation Division, Ministry of Resource Development and Information Technology;

6. Registration fee of RM100.00 and Annual Subscription fee of RM120.00; and

7. 1 Corporate Profile.
